Epidemiological surveillance of children 0-2 years: Asur “tests” mothers

ANCONA – Interviews with all mothers at vaccination centers from this month and throughout the duration of the study. On the starting line, this new initiative is coordinated by UOS Epidemiology – UOC Environment and Health in the Prevention Department in the large area 2. The study covers the first 1000 days of life (between conception and second year of life), which represents the phase where the foundation is laid for development and psycho -physical health throughout the life of the individual and the phase in which early health promotion efforts are fundamental so that the “adults of tomorrow” can fully develop their potential. Therefore, in these days of June ” Supervision of children 0-2 years‘and the contextual “Parenti Più” campaign in our region; The Marche Regional Health Agency has identified the coordination of the course in AV2’s epidemiological unit. With the cooperation of the heads of the prevention departments and the commitment of the operators of the vaccination services, all the vaccination centers in the five major areas of ASUR will be involved.

The project

The purpose of the epidemiological surveillance “Children 0-2 years of age” monitoring the parents’ level of knowledge on the correct behavior (health determinants) to be performed in the baby’s “first 1000 days of life”, such as: intake of folic acid from the periconceptive period, breastfeeding, intake of tobacco and alcohol during pregnancy and breastfeeding, placement of infants in the cradle, reading aloud for babies and their not exposed to screens, safety at home and during transport by car and the intention to subject them to the vaccinations prescribed by the vaccination plan. The Ministry of Health and the Istituto Superiore di Sanità started a pilot project with some regions based on the program “Parents Più” in 2013. In 2017, ‘0-2-year-old child monitoring’ is included in the decree of the President of the Council of Ministers of 3 March 2017, which identifies the monitoring systems and registers of national importance that each region must activate.

ASUR Marche and the Regional Health Agency have joined the road from the beginning, and Vasta Area 2 has positioned itself as the regional leader. The Epidemiological Operational Unit has carried out the drive action by coordinating the previous versions of the Surveillance and participating in the national technical table. The data are regularly published on the website of the Istituto Superiore di Sanità (https://www.epicentro.iss.it/soruardia02anni/), where all the scientific articles and reports produced are displayed. Each edition ended with national and regional conferences open to all health and social workers involved in the field of early childhood. The initiative is also linked to the 2020-2025 regional prevention plan, which includes the “First 1,000 Days of Life” program. Constant analyzes carried out according to the parents’ socio-economic characteristics (income, education and geographical origin) have highlighted that the most disadvantaged groups of the population are also those who are most at risk of not implementing the correct behavior.

2018-2019 edition

The final report from the 2018-2019 edition of 0-2-year-old child monitoring in the Marche region shows that 92.4% of mothers took folic acid during pregnancy, but only 18.5% did it correctly, i.e. from before conception to the third month of pregnancy. 5.9% of mothers reported that they smoked during pregnancy and 28.2% reported that they consumed alcoholic beverages at least 1-2 times a month during pregnancy. 39.9% were exclusively breastfed at 4-5 months of age, while 36.4% of infants continued to receive breast milk at 12-15 months of age. Children who spend time in front of TV, computers, tablets or mobile phones are 29.1% under 6 months and 68.0% over a year, while they have never read a book in the week before the interview, 38.2% of children aged 6-12 months and 25.1% over 12 months. 27.6% of mothers put their baby to sleep in a different position than the recommended one (ie on the tummy), and 26.9% said they had difficulty using the car seat in the car. The use of health personnel for accidents at home that happened to the child affected 16.6% of the mothers.
